Program Underwriting
For All Lines
The "Gold Shield Advantage" program applications must be completed for
each line of business submitted for all security risks. An ACORD application
may be utilized for all classes of business other than security accounts.
We require loss history for the past three to five years, along with current
payroll verification (IRS Quarterly Wage Form 941) and a copy of their
current declaration pages verifying existing coverage. New entities, while
generally not acceptable for workers' compensation coverage, may be accepted
for general liability coverage.
Broker of Record
Letters
In the event that we receive applications on new business from two or
more Producers, a broker of record letter can be accepted. The Producer
who first submitted the risk will have five days to attempt to obtain
a rescinding broker of record letter. A mid-term broker of record letter
would not affect commissions paid to the original Producer. The new Producer
would begin receiving commissions at renewal. We value new business submitted
by Producers, however, broker of record letters cannot be accepted on
"in-house" accounts already submitted to the carrier or written in the
program.

Sub-Brokered Business
A great deal of information is required from Producers to accurately evaluate
an account. It is imperative to obtain this information from the Producer
working directly with the insured. Therefore, we cannot allow our Producers
to sub-broker business. Any exceptions to this rule must be approved in
writing by Izzo Insurance Services, Inc.
Certificates
After we have issued a binder, Producers have the authority to issue certificates
of insurance for their clients. Any certificate containing modification
of standing wording, additional insureds, waivers of subrogation, etc.
must have prior written approval from Izzo Insurance Services, Inc. The
certificate of insurance is NOT to be used as an endorsement request.
Any policy changes must be requested in writing.
